Your first visit to an oral surgeon can be intimidating, we're here to take away the mystery and make it a pleasant experience!

Please bring with you to your appointment:

- The referral slip from your doctor or dentist
- X-rays if applicable
- A list of any medications (or supplements) you are taking
- Your medical and dental insurance cards
- Print, fill out and bring with you any necessary Patient Forms

Please alert the office if you have a medical condition that may be of concern prior to surgery or if you are taking any medications.

Please request that any X-Rays be forwarded to our office. If there is not enough time for them to arrive by mail for your appointment, please pick them up and bring them to our office.

PLEASE NOTE:
All patients 18 years of age or younger must be accompanied by a parent or legal guardian at the consultation visit and surgery appointment.
