The referral process typically begins with a general dentist, or other referring specialist. Once he or she determines that an oral surgeon should perform the required procedure, they will complete a referral form and send it to our office. You should also receive a copy of this form for your records, and should contact our office as soon as possible to schedule an appointment. Your first visit to our office (Consultation) will be to determine your course of treatment. Upon completion of an examination, a review of your x-rays and a detailed medical history, you will have an opportunity to discuss the proposed treatment, necessary anesthesia, and your recovery with the surgeon. It is important to us that you have a good understanding of the treatment we will provide. To help educate & provide guidance through your procedure, we will provide you with an interactive program that you can view online in the comfort of your own home. Please bring your x-rays with you on this initial visit if they have not already been sent by your dentist. Patients under the age of 18 years must be accompanied by a parent or guardian at the time of examination. Patient Forms |
